Seatbelt not secured around 6-year-old who died falling from adventure park ride

A 6-year-old girl who fell 100 feet to her death from a mineshaft-themed ride at Glenwood Caverns Adventure Park in Colorado was not properly secured in the ride, reports invesigators.

Investigators found the girl, Wongel Estifanos, was sitting on the two seatbelts instead of wearing them across her lap, and the ride's two newly hired operators never noticed even though they made seatbelt checks.
